Urology Care Clinic services are not available for medical emergencies. If you have an urgent situation, proceed to the nearest emergency medical hospital in your area.

Your consultation is recorded for future references and your document might be stored on our server. You will be asked to confirm your identity by providing a copy of your ID, email address and phone number.

Online Services

Urology Care Clinic uses the newest technology to hold a virtual meeting by appointment with Dr. Khalil Armache. The face-to-face appointment is a real-time, private and secure between the doctor and the patient.  The online services do not replace the patient visit to the doctor. However, it is a complementary and a convenient service provided to our patients. The service is not designed for emergency care.

Payment Conditions

Urology Care Clinic charges for the online services when the patient books the appointment. Our communication with you starts when you start using the services via our website. You may use your laptop or your phone to conduct the face-to-face communication with the doctor. We use ZOOM as our method of online meeting, you will need access to the internet. The meetings are set by default on ZOOM for 10 minutes per consultation.

Acknowledgement

By accepting our terms and conditions, you authorize Urology Care Clinic to charge when booking the appointment. Our terms and conditions are our bound to comply with you when using our online services.

Online Appointment

1. We assume you are the person who used our online services to register for the virtual appointment.

2. You agree to protect the privacy and security of the Doctor, and employees. 

3. Urology Care Clinic do not attempt to capture images or take screen shots however we do record the online meeting using ZOOM for future references.

4. We commit to guard your privacy and safeguard your medical records.

5. You cannot use this service to request any electronic medical records stored with Urology Care Clinic.

6. You commit all information provided to Dr. Khalil Armache is yours and is used for the soul purpose of this consultation. You are expected to provide the Dr. Khalil Armache accurate information and do not omit any relevance information. You may not in any shape or form provide any incorrect information.

7. The Doctor’s advice is based on your accurate information you provide us. The Doctor cannot be held responsible if you provide inaccurate, false or omitted information.

8. It is your responsibilities to provide the latest medical records, images and test results to the doctor at the time of appointment.

9. It is your responsibility to maintain and ensure your personal and health information is current and correct at the time of the consultation.

10. It is your responsibility to comply with the Doctor’s instruction as advice.

11. It is your responsibility to report side effects of any prescriptions to the doctor.

11. When in doubt or have any concerns regarding the advice received from the doctor, it is your responsibility to seek another medical opinion from a registered doctor.
